Dog Adoption Application

Thank you for considering a rescued pet as an addition to your family. Please note that our application is brief, but is an important part of helping us match pets with families. We suggest that you have the following information available before you begin:

  • The name AND phone number of your vet (if a former pet owner within the past 5 yrs).
  • Approval from your landlord if you rent (you may apply if you are still awaiting a response from your landlord).
  • If you live on military base housing or if this is a possibility in your future, please note that there are breed restrictions in place. Take this into consideration when applying.
  • Applicants must be 21 years of age or older. Anyone under 21 years of age will require a co-adopter. *Please do not begin this application until you have the approval of a co-adopter if you are under 21 years of age.
  • If completing this application on behalf of someone who requires assistance using a computer, the information must be the information for that person (the potential adopter), not the person completing the form (you).
*We check vet references and landlord references (if applicable) before further processing applications and contacting the applicant.*

Our listed pets are located in the Greater New Orleans. Please note that we do NOT ship animals, and out-of-town adopters are only approved on a case-by-case basis and must provide transportation if approved.

The questions contained within this application are not meant for the sole purpose of excluding an applicant, but is one of several means of assisting PAS in identifying the best match for each applicant and animal. PAS reserves the right to refuse adoption to any applicant.  PAS does not share any information submitted in an application without the consent of the applicant, except for in cases where information is found to be falsified and/or which PAS deems to put at risk one of our animals or animals in other rescues or animal shelters. Adopting a dog or a puppy is a serious responsibility.  A dog that you adopt today will likely be a part of your family for the next 10 to 15 years, and perhaps more. A dog makes considerable demands on your time and resources. As a dog owner, you will need to spend time training, socializing, grooming, feeding, scooping poop, exercising and playing with your dog each day. Puppies and dogs that require further training need even more time and socialization. All dogs require regular medical care. Some dogs may eventually require special food or medications. Some dogs require regular professional grooming. Vet bills, pet sitting and vacation boarding fees may add considerable expense to your family budget.

By signing below, I certify that the information entered on this application is true and that knowingly falsifying information will result in the denial of said application. I also understand and agree that submitting this application places me under no obligation to adopt any animal from Pet Adoption Services, Inc, nor is Pet Adoption Services, Inc obligated to approve my application or adopt any animal to me, and that the applicant(s) and/or Pet Adoption Services, Inc may discontinue or withdraw from the application process at any time.
I give my veterinarian permission to release any and all health records of my current and past pets.
